"Regions can only belong to one duchy. Every region should belong to a duchy. Regions not belonging to a duchy can not have a region lord because only a duke has the authority to appoint a lord."

If only a duke can appoint a lord, than how does a region get the first lord whom aligns himself into a duchy?
