You added the unique item information to the wrong page. Create a new page under the name of the item and put the template on that page. You can do this by typing the item name into the search bar on the side, it will say the the page doesn't exist and give you a red link to the page so you can create it. I have removed the changes you made to the help page and will give you the exact text you used below this.
